What's the most innovative city in the world?
I've visited some great cities and it's exciting to see different approaches to fostering innovation. I'd be interested to hear what others have seen and your views of some ways cities are promoting innovation.

Boston or SanFran for startups that go big.
Manchester as the best city from a tax perspective in europe.
Kansas city for its Kaufman foundation and rippling effect there for entrepreneurs.
Vancouver as the greenest city and one of the constently best cities to live in the world.

On track record alone the current holder still has to be Silicon Valley.
The answer would depend on the type of innovation. It is a mindset trap that we all get tempted into thinking that innovation = technology. More specifically computer or electronic technology. Innovation occurs in every industry including "low tech" spaces such as food, music, performing arts, fashion, art, urban design, and education. Where do the guru's in a specific field go to be inspired and energized -- that would be how I would define most innovative.
I would say - Silicon Valley in US & Berlin in Europa, considering the support for startups and innovation.
I would say Georgetown, Penang a northern state in Malaysia. The state was innovative in creating a free trade zone and a major semi conductor producer in Malaysia.
Tel-Aviv; just choose a random coffee shop and you'll find at least two tables where bright eyes entrepreneurs are pitching to a glazed eyes investor :)
Bangkok is rising fast. I've been here for 7+ years, started my company about 5years ago. The past year or so the pace of things is really picking up.
First 3 Day Start-up coming
Things are not so expensive here, there's lots of talent, and a growing community across tech and design.
